5 Healthy Towns

The 5 Healthy Towns Project (5H) is a groundbreaking, innovative project that involves planning and funding of a community-wide wellness plan. The goal of 5H is to create the healthiest five communities in the Midwest. The Grass Lake Community Wellness Initiative (GLCWI) and the Chelsea-Area Wellness Foundation (CWF) are working together on a wellness plan that incorporates existing programs and new strategies to impact community wellness.
The initial rollout was followed by collaboration with the GLCWI to maintain and expand wellness-related programs, policies and infrastructure.
The 5H is comprehensive; impacting everyone by focusing on a variety of wellness related activities. It centers on four elements that are the core of CWF's vision:
- Eat better
- Move more
- Connect with others in healthy ways
- Avoid unhealthy substances.
Community representatives provide an understanding of the community, existing resources and needs. The CWF provides knowledge of what's been tried and proven effective, nationwide. There is room for innovation if the probability for success is high.
Together we can create a culture of wellness and foster sustainable improvements in the health of our community.
